Friends, I have a question. Will appreciate quick replies. I have been asked to provide reference letters from my past employers. I talked to one of my past managers and he asked me to frame the letter and send it to him. He said he will sign it. Now I have one question:

How should the letter look like. Should it be like " TO WHOM IT MAY CONCERN" or should it be addressed to "Canadian Consulate". About the responsibilities, should it mention each and every responsibility or mentioning that "He worked as a software  developer" is enough. Please advise.

Can anyone share a sample letter.

1.  Addressee either or, it really doesn't matter.
2.  Must outline the job duties, not the just the title.

Thanks Friends...When you say that letter must outline the job duties...what kind of detail it requires...

Should I say that "He worked on software development" or does it want to go into details such as ""He did coding in Java" . Please help.

Also, I need to mention salary when leaving the company , right ? Not the salary when i joined, right ?

both of your job descriptions sound good. So, first mention the title of you position, then some job duties. For example, responsible for coding, application of Java etc.
For salary, mention your last salary or mention as an average salary. For example, his monthly salary  from the company was .......... per year.

In addition, take the letter on an official letter head and if available attach a business card of the person (your boss) who is issuing you the reference letter.

Do mention the job duties as you mention on CV's.Also mention the salary when you were leaving and not when joining.
