Hi All,

I have filed my papers from Buffalo under 0213 and recieved medical request from them yesterday for me and my wife. But I am blessed with baby in Aug, 2011 who is not part of application process yet. I intimated buffalo office via email which they acknowledged too. My baby is US citizen, being born here. Please let me know if anyone has similar case and what should I do in this case. What is process to include her in application process now and how to get medical request form for her. Should I submit my medical first(as it need to be completed in 60 days) and send her details/fees along with it?

My status and dates are:
Sent application to CIC - Jan 2010
Received response from CIC - Apr 2010
Submitted all documents to Buffalo - June 2010
Received email from Buffalo office - Nov, 2010
Received medical papers from Buffalo office - Nov, 2011

Current Status in CAS - In Progress.

Hello Jatz
Below is the detail of documents or actions that you need to take in order to comply fully with CHC requirements for new born;

1. NEW BORN Birth certificates (hospital and ministry of health). No need of notarization. Only copies can serve the purpose.
2. Passport of new born (and visa page if residing outside homeland)
3. Five photographs as per specification,
4. Additional family information forms (update) and signed in original
5. Demand draft of CAD 150/- (in the name of RECEIVER GENERAL FOR CANADA)
6. Information on height of new born at the time of sending forms
