Hello Everyone,

Just a couple of questions:
1)Do I have to go to the same doctor as my partner? We are in a long distance relationship so I would have to travel to do so.
2)Does the medical form state who is the sponsor and your relationship to the sponsor? We are a gay couple and my partner is in Venezuela and I am scared about the doctors reaction. I have seen several 1017's and I am not sure about which one we will get from the consulate.

There is a tick box with relationship to sponsor - options are "Spouse, common law partner, Conjugal partner and dependent child".

The IMM 1017 that I am looking at is (02-2005)
