Quick question about medical exam for PR

Heya, just waiting on my marriage certificate to file for PR. I haven't gotten a medical exam yet, but I have the Section A form.
Now, when I get the exam done, do I get the form back and send it in WITH the PR application? Or does the doctor send it in himself? If the later, should I apply for PR and then get the exam done, or does it matter? Just don't want it getting lost or mixed up or something. And I also need to bring photos for the exam? Thanks!
