Questions about additional family information form

Hello friends,

I have one question about the additional family information form. At the top of the form it says that I need to write all names in English and my native language. Then what about other items such as Place of birth, Maritial Status and Present address? Do I also need to write these items in both English and my native language? Or just in English?

HOW WE DID OURS WAS WE FILLED IN OUR NATIVE NAMES AND ENGLISH NAME.WHAT THE MEAN IS ALL UR NAMES BOTH ENLISH AND NATIVE SHOULD BE FILLED IN  THE SPACE FOR NAMES .

FOR OTHER THINGS WE WROTE IN ENGLISH.I HOPE THIS HELPS
