Question on Background Declaration - Schedule A- IMM 5669E

Hi,

I am applying under the skilled worker category and have a couple of questions regarding the form title Background Declaration - Schedule A- IMM 5669E.

1. Questions 9, 10 and 11 ask about membership in a professional organization, involvement in government positions and military service respectively. In each case my answer is "NONE", but it isn't clear on which box I should write the word "NONE" (or "N/A") as the form provides not boxes but tables for the above mentioned questions.


2. In case of personal history and residential address, what date do I enter in the "To" box while writing my present occupation and present address? Should I hand-write the word  "PRESENT" in each case?
