See number 19 field on the personal details section in the official guide here: http://www.cic.gc.ca/english/information/applications/guides/EG74.aspAs per the instructions above, we do have to submit the CNIC translated (CNIC from NADRA is in URDU) / certified true copy for the principal applicant at least.

Adnan,
Please clear what exactly you need to know. That Nazim thing is not that hard if you want to take Birth Cert for your KID, Even if your passport have NIC# on it you must have to let them know you are holding ID card as well. Please refine your requirement. Hopefully we will reach to right answer.
Regards,
Jawad...
Jawad I'm single, I live out of the country, hence Nazim is out of reach, I've the new version of NADRA'a Passport, and a digital ID card which at the moment is in Urdu but getting the English version should not be a problem.

luckly i got the photo copies of my certificates, which helps a lot. inter is very easy but matric is difficult, cauz you need to go back to the school for attestation and date of birth verfication, beside this you need a police report and advertisment for the lost certifcate should be published in local media. actually i m abraod my brother did all this for me. i you want more help you can contact him. what is the procedure of getting duplicate matric and inter certificates? My brother has lost his originals

Adnan,
Do you have any proof of your birth? If not birth certificate, then there must be any proof from hospital or committee office at least. We call it Janam Parchi as well in Pakistan.
Thats not compulsory to see Nazim and all if you have any other proof you can do it by sending someone to committee office. You must have to arrange that Birth Cert. Do not worry. Its not a big deal, but still some papers are needed for this.
For me, i don't have Nadra's birth certs which they are issuing now a days, i have a cert which is attested by notary public office. Tell me do you have any slip or any thing? if not then ask your any relative in Pakistan to visit Oath office and explain them detail and show them some proves and attestations like your addresses and IDs that you were having in Pakistan. They will issue your Birth Cert for sure.
Hope you got my point. Still you can ask on above.
Regards,
Jawad...
