I have a quick question

The Biodata page required during submission for CIO, does it has to be..?? :

1. Only the 1st page of the passport which has the Name, Date of birth, Passport issue and expiry date and photo.
2. 1st and Last page (which has father's and mother's name with address etc)
3. An additional  sheet on which all this info(Passport No., DOB, Passport issue/expiry date etc) is written.

Seniors please advise...

My passport country has only my biodata, not my father or my mother. It is only one page.

Hi Yarla7bh, is your passport an ICAO standard passport? An example for an ICAO standard passport is given in this image http://upload.wikimedia.org/wikipedia/commons/7/7e/Mrp_image.gif

If so, you need to submit only a photocpoy of this page. If your country has a different type of passport, then I suggest to submit copies of pages which have any important information.

Thanks for the info.  I have an Indian Passport, the first page has my BIODATA details, it does not have the machine readable code on it.
Last page of my passport has name of Father, mother, address.

My question was do i need the First page and Last page or  Just the first page.
I guess i will include 1st and last page just to be on a safer side......

Could anyone let me know if the BIODATA photo copy needs to be NOTARIZED...?

I don't know what an indian passport looks like, but you don't need father's or mother's details in the passport for CIO. In my case (Sri Lanka passport), there are no details about parents. Not even the address is mentioned (addresses can change).

If its only father's and mother's names and address, then it is actually not required, but if it had some important info (birth place, birthdate for example) then they will need it.

To be on the safe side, send photocopies of both pages.

YOU DON'T NEED to notarize the PP copy.
