question about personal history

my wife recieved a letter and is say awnser personal history til [ july to present but it has pen written on itf rom the officer] , then in the buttom it says addresses [ without pen written] do i need to awnser that question too or just the officers question only?

Is it a form letter with various choices, with a box next to each choice? If yes, you only need to provide the information for the choice that is checked.
However, I am not quite sure what you mean, so the safest course of action would be to provide the personal history information
and
the address information.

As your wife has got a letter requesting her additional information, she should furnish all details asked by the visa office,whether it is type written or hand written.
