Icant really comment on the picture questions but i have asked a few times about the medical and im pretty sure im very clear now:
(before i answer, know that im Canadian and my husband is American, we are applyingas a spousal sponsorship and outland - so similar to your situation)
Based on everything i read and questions i have asked i strongly suggest that you wait to submit your application until you have your FBI letter and your have undergone your medical.
FBI takes approx 8 weeks (when did you send it) and its a requirement based on the guide that you submit both the FBI letter and the Appendix Cwith your application. Holding off and waiting for further instructions could result in your application being sent back incomplete or delaying the entire process all together.
I know the guide and all the paperwork is so much to read but it really means so much to read every word, small and large print, they might slip something in at the bottom of a page that you over look but could really have an affect.
Conclusion and my opinion: follow all instructions and do not leave anything out, this forum is amazing and asking questions is a really good way to gain confidence before submitting your application.
Lastly, i too was told that i would be sent a form but after re-reading the guide and asking questions here it appears that the DMP was probably confused and didnt realize we were applying as a spousal sponsorship and its a must that the medical be submitted with the application.
Like some have shared with me, if you are getting wrong or unclear advice try calling a different DMP and see what they have to say.

I live in BC. Wife lives in Washington state.
I sent the FBI almost a month ago. I did attempt to call and make an appointment but the receptionist basically said the sponsor\applicants normally bring a form that they fill out and after they're done, they ship it straight to the CIC. I did not mention it was a outland spousal application though. Although they did know it was for a spouse.
I will give a call tomorrow and try another one. Have you actually gotten your medical done then? Did the DMP provide you with the forms you needed? Once you passed, did they sign off on a sheet? Lastly, did you need photos? What size?
Cut off the white. You are supposed to submit the medical WITH the application. The form to fill out is Appendix C of the Region Specific Guide. Take that to the doctor. And you are supposed to submit the police clearance with the application. But you can write in a cover letter that you have applied for it and will submit it when you receive it.
When we're done the medical check, do they provide us with a piece of paper right there? Or do we need to wait for results in the mail?
