Question about CSQ (!)

At the "Generic Application Form" under "application details" part, 7th point, it is explained that if I check "No" for the "Have you received your CSQ?", I must indicate the date when I applied for it. But I haven't applied (and not intend to) for it.
What should I do? Leave it blannk???

If you check ''no'' the space will blank out automatically.
