Query regarding Supplement form

Can anybody explain me what to write down in Supplement info. form. I am an Indian citizen and currently living in UK for last 4 years. Do I need to enter all trips made by me other then my country of origin(India) and UK (Country of residence)
OR

All trips trips even to my country of origin while living in UK?

You have to list all the international travels incuding the country of origin and current resident country.
