QUERIES REGARDING PR APPLICATION

I have a no. of queries
1. iam a dentist from India and both me and my wife are post graduates in dentistry.
We have about 4 years experience as faculty in dental college which is attached to a hospital. Though we treat patients occasionally, our main duty is training undergraduate and postraduate students.But I also practiced general dentistry for 2 years in the evening in my own clinc, but filed IT returns only for a year for clinic, is this proof enough?
2. will evening practice be considered as part time job?
3. if i dont get reference letter from my employees for my work in hospital instead only as faculty will there be a problem?
4. Can anybody help me out with format and matter for recommendation letter for dentists?
5. confused about whether i need to use a consultant, I have aproached Abhinav consultancy, but not able to make up my mind
kindly help me soon as time is running out
