Im a caregiver program, Im getting married. how long the process inside canada for spouse? did i need to go back home?

You don't have to go back home at all nor do you have to do an inland application.  What is your nationality?

Are you a PR yet?  If you get married, you include your spouse in your application for PR when you are eligible to apply.  LIC PR applications are taking about 18 months.

Oh you're a caregiver but you've met a Canadian and would prefer he sponsors you in a family class application?
