Problem filling up Sch 1-Background declaration

Hi everybody,
I am trying to fill up the Sch 1 -Background form for FSW category ( IMM 0008 (01-2009)E , but for questions related to date of birth esp for the 'month' and 'day' is showing up as "TRUE" instead of the original numbers (on the screen and after printing). Same thing is happening with the form for Payment of fees wherein details of the "Expiry dates" are required.
Anybody faced this problem...how to rectify?

Leave the items blank and write the details in.
Download and annotate the form using a PDF editor.
