My wife has been doing her Master's course in Italy. Does she have to provide a Police Clearance Certificate for the period she stayed there (more than 6 months)?

Even if she is not the principal applicant? She is the dependant (the spouse of the principal applicant)....

Yes - she must provide it even if she's not the principal applicant. All applicants (both principal and dependant) must pass the security check to qualify to immigrate to Canada.
