pls help-statutory decleration for non accompanying spouse!!

Dear All,

Could you please advise me how to write a statutory declaration stating that my spouse will not accompany with me in Canada? 

It would be a great help, If you can show me an example or can write a format here of that declaration.

Do I need to send any other documents with that deceleration?
