PLS HELP,,About APPENDIX A

hi good day.. I just received the letter from the embassy with regards to medical requirements.. so we're now about to be medical examined. my question is. there is just a letter from the last page that we are required to complete the Appendix A of myself and my mom. and submit the original passport of me and my son... What and where can i find the form of Appendix A... it said here that it is attached here, but there's no form that is being attached in the letter.. thank you.. hope u can answer my question
