Pls Help! Manitoba Provincial Nominee Program

Hi everyone.

I've been reading this forum for quite sometime now though this is my first post.  My husband and I are planning to apply for MPNP - General Stream.  I have relatives that are willing to sponsor us, my husband will be the principal applicant.  Please somebody in the forum help me with my queries.

1.  My husband being the principal applicant already has IELTS.  Do I still have to take IELTS too? (me being the dependant).
2.  In Generic Application Form for Canada (IMM0008), there's a question for UCI (Universal Client Identification number).  We applied before under FSW1.  Unfortunately, we receive a negative eligibility review.  In the letter sent to us by CIO, a UCI was assigned to us.  Should we indicate this UCI in our MPNP form?

in form of imm0008egen (application for permanent residence in Canada) and in family details information on page 2, http://www.cic.gc.ca/english/pdf/kits/forms/imm0008egen.pdf path they have asked about knowledge of English or French for dependent family  

if the question “ Can communicate in English or French” is ticked as YES, so the MPNP office would ask for prove otherwise they would not ask, And for sure they are going to ask from principal applicant
And you have to be honest in filling of Generic Application Form for Canada, because they have the record of refused application

Hi haroon.  Isn't this form (imm0008egen) for federal skilled worker only?  I looked upon the forms needed for MPNP, and this form is not included.

I did a self-assessment for MPNP General stream just to know if we get the sufficient points.  There are 5 factors: Age, Education, Work Experience, Language and Adaptability. 

Under Adaptability, we fall under the family-like support from distant relative or friends in Manitoba criteria.  My relatives who will sign the MAS form are husband and wife.  Am I correct to give us 5 points?  It is not mentioned in the PNP kit that husband and wife are not allowed to sign the form together.  It only says to provide complete MAS form from at least 2 people known to you and who live in the area to which you are destined.

Any clarification about this one is highly appreciated.

i got my nomination from MPNP under family support stream, they have asked for IELTS from me but did notfrom my wife because i have already indicated that my wife can not communicate in english.
beside if you see the check list they did ask for the form which i mentioned b4, its mandatory to be attached with your application
