So my husband picked up an envelope from the visa office today and it was a request for additional documents....we dont know how to get 2 of the documents.

What is "REASONS FOR DECISION AND ORDER - REFUGEE DECISION BY IMMIGRATION AND REFUGEE BOARD / PERSONAL INFORMATION FORM"
      My husband applied for PR on humanitarian and compasionate grounds and was denied.  Are they asking for the letter from the Pre-Removal Risk Assessment Officer that my husband got explaining why his PR was denied??? What about the PERSONAL INFORMATION FORM? I found in all my husbands papers from over the years a Personal Information Form all filled out from 2005 when he applied for Refugee. Is the VO requesting this form??

Also they want "DECLARATION REGARDING PREVIOUS MARITAL STATUS WITH XXXXX XXXXXXX" my husband was not married before me.  What is this?? How do we get it??

I only can answer the question above. In my country it means " Certificate of No Marriage " or "Certificate of Singleness". It is being issued with a fee by the National Statistics Office (or Census Office)in our country and we can request it online/internet from our country.

Or any original document that will show that your husband was not legally married with other woman before he married you.

Well, my husband called the office and they said that they have never heard of issuing a certificate to state that he has NEVER been married! this is SOO frustrating! Angry The only thing we thought of is to call the place where "cedulas" are issued and maybe they can direct him to the right place? Anybody else have any ideas??

Much better if your husband can get the document from his country. But if not, a "Statutory Declaration" stating that your husband is not legally married to anybody before he married you.. You can get this "Statutory Declaration" from a Commissionaire of Oath in the area/city your living now. And a document (like cedula; drivers lisence) that shows that your husband is single before he arrived in Canada...

we think we found out where to get that certificate to prove he was never married. He went to get his police records re-done today and they told him to get that document from the office that issues cedulas...hopefully that really is the place to get it! Thanks for all your help  Smiley
