please help me out. i'm confused

I became PR in Calgary cic office under family class and I need now to apply for the citizenship and I only have PR card and based on the cic requirement, I do not remember I was given the IMM 5292 or IMM 1000. And the question is do i need to apply for these documents or can i apply without them?

also I tried to apply for the imm 5292 but in the Form IMM 5541 the imm 5292 is not mentioned. Only the following documents were listed. Please help me which document I should chose.
