Hi to all,
         I would like to order  Caip Notes. My friend in Canada wants to order it on my Behalf.

Questions

1 How much do i have to pay?

2) what papers do i have to fill and what papers do my friend has to fill.

I read about consent letter, do i have to fill it or my friend has to fill it?


Please help explain the process to me. It is my first time to order Caip Notes.

You don't have to go through agencies for $35 or more. 
I ordered CAIPS notes onbehalf of a friend for $5.00 through Access to Information and Privacy Division, 360 Laurier Avenue, West, 10th Floor, Narono Building, OTTAWA, Ontario K1A 1L1 
They gave a website: www.cic.gc.ca/english/department/atip/index.asp 
Client Service Line 613-946-0953
Our friend hand wrote a letter, signed it and faxed it to me and I enclosed it with my request and cheque.  Sorry can't recall who the cheque is to be made out to, but it must be on the website.  Good luck.

AHA!!!  Thought of looking in my cheque book and what did I find:  cheque is made out to The Receiver General of Canada and it took exactly 6 weeks from mailing my request to getting the Notes.  Hope this helps.

Had to go and look at the link again.  The Canadian fills out the request and the person whose CAIPS have been requested writes a letter authorizing the "Canadian" to request them on their behalf.  Fax or scan and email this letter to your "Canadian" to enclose with the completed form and $5 cheque. 

The letter addressed to Access to Information and Personal Information should state date, your name and address, birthdate, passport number and give authorization for the CAIPS NOTES (or whatever information you are requesting - see the check list) to be released to  XXX (the name of the Canadian).  Your Signaure. 
Hope this is clear.
