In what form are these PCCs from USA (fbi; and states like illinois, newyork, etc) issued?

Do they just send back the finger printing card with stamp (say no criminal record found), (or) do they issue a separate certificate, (or) would it just be a computer print of the record from their sytem?

The FBI returns the fingerprint card with a stamp on the back (no record) - if you have no record.  (I have no idea what they send if you DO have a record). 

As for the states, each state is different.  I mostly just received letters.

Most of the states will send a Letter stating you dont have a criminal record..
Some states will send back the fingerprint with a stamp on it.

Differs from state to state. (Like I got 2 different formats from CA and MI)
