Hi All

Interested in hearing back from anyone else who has been in this same situation.

Received my med request 2 weeks back but the photos have been inter changed / incorrectly pasted on the medical forms sent by CIC. Just goes to show that no matter how sophisticated or advanced you think a country or its services are , they are still prone to making human errors  Cheesy Have contacted CIC a few times on the email provided on the covering letter alerting them of the error but have yet to hear back from them.  Angry

Just wondering if someone else out there had this same issue and if CIC did ever respond to your email / fax alerting them of the error or if they just resent you corrected forms. How long did it take for CIC to resend the new forms ??

There's nothing to worry. This error can be fixed by your DMP itself. They have a guideline to that effect.

Procedure:
1. While booking the appointment with your DMP - inform him/her of this issue.
2. Carry additional set of pics (CIC size).
3. The DMP verifies them thru your passport records.
4. Clips the correct pic aside the CIC pasted one & certifies* them.
5. S/he also writes about the change on his/her report to CHC.
6. Upon your return home - send an email to your VO informing the foregoing.
And voilà, u r fit to go!

*A crook DMP might charge u extra for this service (just a couple of $$ additionally).

Just an update on the outcome of this query for future reference should someone else encounter something similar.

I sent a weekly email to my visa office pointing out what had happened and my 3rd email finally did the trick.
I got a response to the effect that they will resend new forms with photos corrected and that my deadline is extended for a further 30 days.

I scanned the forms with the mistakes , circled/highlighted the errors and submitted it along as an attachment on my email advising them of the error.
