about the UCI#, if the letter that CIO provided stating cap reached, have any then u should write it down but if not then there's no need. about POF, it's strongly recommended to have the amount in PA's account not others. though at later part they will ask for PSNW where you have to provide all the bank account statements and net worth of both PA and accompanying spouse. |
