Permanent Resident Authorization form, CIC NY.

Hello All,
 Please can someone who had his PR processed in CIC New York Help. I have a quick question, I was just lately trying to recall the information we were needed to provide in that Permanent Residence Authorization form. I guess there were: Height, Eye color, Name of Applicant and Dependants. Did they ask for date of birth? I can't remember again if DOB was part of it. Pls respond. Thanks for your responses.
