It asks for my original entry to Canada date. Is this the date in which I first entered Canada ever? or the date in which i crossed to come and visit my spouse?

First entered ever.  If you can't remember the exact date, put one in and after printing, draw a line through it and handwrite "unknown".  Explain in your cover letter that you cannot remember the exact date of your first entry and give an approximation.  If you are doing the online application, write the closest date that you can remember.  Write a letter and upload it as an attachment to one of the required documents, i.e. passport.

Thank you. do you think my wifes pay stubs are enough proof or does she need a letter from her employer too?

Give them what they ask for which is a letter from her employer.
