I don't recalling filling up any forms. We went to DMP and they then did the test and doctor asked questions, thats all. This was in 1997.

It is the same now. The only difference is that CIC will send you a form (filled already) and you should take that with you to DMP.

you (your parents) will be given a form with your photo, you give the form to a doctor, the doctor is supposed to ask all the questions and fill in the form, then you sign the form confirming that it's true...  you are not supposed to fill the form yourself... but you are signing it...

Passport is not required for medical
hmmm.... in our case we were told to bring a passport...

Please bring the following to your medical appointment:

·Two recent passport-size photographs of each family member.  Refer to our website for photograph specification:  http://www.cic.gc.ca/english/information/applications/guides/5445ETOC.asp#appendixB
Eyeglasses, if applicable; list of prescription drugs taken, or any relevant medical report(s).
Original passport of each family member.

Same in our case, we were asked for passport as well, seems to be exactly same document as mentioned above.

Passport is required (even within Canada when you do medicals for immigration). However, I know that in Canada, for example, you can substitute the passport with another identity document with photo. Ask the doctor who will be performing medicals. It all depends...not only from country to country, but from doctor to doctor as well.

My parents took an ordinary photocopy of passport and DMP accepted that.

Passport is not required for medical
I think it depends on the DMP, in my case, my parents were asked to bring original passport (to verify) to DMP office and a photo copy of it.

I see a user named BHUV2004 whose file with a dependent was sent to CPP-O. Is this a mistake or it is possible?  My understanding is that all the files with dependent are transferred to local visa offices.
i have the same question Tongue
