I dont know where r u living. If any big city e.g. lahore go to any court there are offices for notary public, oath com and they also translate marriage certifiate.

First make the translation then translator will make an affidavit and oath commisioner will attest it.

If you want more secure then go MOFA office it is available in lahore and islamabad, just take original marriage certificate and translation they will verify it but it is additional not compusary.

As suggested by Guddu  go to nearest Court office, ussualy the notary public have the forms printed they just need to type in you details and then  attest this, and u r good to go

send me your email addressed i have format. Recently i send it AEO so you can fill it that in excell sheet then notrized..........asraruaf  @  hotmail.com

My friend it is not enough, first check the requirements for translation on VO form then reply. You will need to translate and then a affidavit is required that it is true translation and an oath commisioner will attest this.

hope this will help.
