I have been asked by CHC-London to submit list of my travel document. I downloaded this form from the website. When I opened it in Latest version of Adobe Acrobat reader, I am not able to fill it.
Can any one guide me how to fill that form

If you have B Form of baby put the B form No. which will be consider ID No.
Please re-confirm it. National identity card number should only be mentioned if someone is 18 years of age. Though CRC [B-form] have same number on it but B-Form is not a national identity card. |

Its been a while and I'm anxiously waiting for a step forward. What does the last line means and where do I stand?

As my e-case shows received medical reports only not mentioning any thing about other documents sent soon after medicals. Please advise
Dear friend, Hold your horses, atleast your med acknowledgement is updated. In my case both things are at status quo  . I would suggest send them an email enquiry by 25th of May 2012. Best of luck. |
