Ok check this link where forum member shibuya said if you are asked for this form it further delays and they put you on further check. Check the following link. If you also see the form they again ask for you full personal history and occupations etc. Why is that when you had already provided that info before? So this is my opinion by submitting this form they further do some checks. Thanks

Dont worry dear Its not a Big deal. From last 4 months mostly applicants are getting Travel Supplementary Request IMM5562 form.

Lets see what will happen in coming few months.

These forms does not means that this candidate is on full security checks.

Dear this is very strange becoz  i have travel history from different countries like INDIA/KOREA, but they didnt ask me to submit this form and marked big crossed on the spouse column.

My wife first time visit in her life and they ask for her IMMIGRANT SUMMARY FORM.

They didnt give me any link for this form they simply send me this single page form with covering letter that send back this form in 30 days.
