Below is the detail of documents or actions that you need to take in order to comply fully with CHC requirements for new born;

1. Birth certificates.
2. Passport of new born baby
3. Five photographs as per specification,
4. Additional family information forms (update) and signed in original
5. Demand draft of CAD 150/- (in the name of RECEIVER GENERAL FOR CANADA)There is no form to fill for payment
6. Information on height of new born at the time of sending forms.

If you are in Lahore then go District katchery. Near Civil Lines college hostel. you will find a big shop. Forgot their name. You can get it translated and notarize all your documents from there. They r very good. don't go in the katchery where you see many desks of translation and notrary. their rates are bit higher and stamps are not good one. rest your choice.

Go to Kachery, you can find "chaudhary brothers translation center". they are famous you can ask from anyone there about their shop near civil lines college.
