I think you have two options here. one is told by "mmiq" and i would like to add more that talk to your DMP now before going for medicals.
and
the other is to seek advice from CHC,London. and write the subject as "Urgent: Medical forms."
This was you may get fast reply.

how many people faced this problem with Dr.Arif? just one ?and started with a newbie like u on this thread,i am not defending anyone but it could b character assissnation of the doctor, trust me doctor cant do anything to anyone after sending medicals,so if even a few people are willing to go to doctor,i will go esp with complainer,
well its not a matter of newbie or oldbie....does it matter...really not bcos regardsless this forum we r grown up matured and educated people...so we can have and express our opinions and there r many school of thoughts in the world and communication is the best tool to solve things and to create peace so its better to address and come up with the real issue
And other thing as u said how many people faced this problem with Dr.Arif? may i ask u 1 thing how many people r on this forum(all applicants from lahore or punjab? how many people have dare to disclosed such incidents? and how many received medicals so far?) no one can answer this accurately.
More, its not the matter of 1 person faced this problem, do u think in such issues...quantity matters....really not
every single person's respect is important as per my view point.
Other thing is prior to accuse someone ... there must be some prove, ok I agree. But just think on other side what advantage an effected person getting from this forum by telling all u guys about a very sensitive personal issue....sure nothing but just more frustration. So why he is doing this because on this forum there is no one patient of that dr. and I am sure he is not dr.'s competitor.
Moreover, issues like this sometimes a person could not have proves so we must consider all scenarios. I think Mr. pk-canada should not go to that dr. again and try to forget this issue because in this country no body accepts his/her faults....from a clerk to prime minister and president. Its not ours tradition. The best solution is be careful

I feel doubt with newbie means someone made new id and started character assasination of the doctor while i have told u that i know dr.Arif for the last 20 years and he could get so many oppertunities in his service as he is full professor in Jinnah Hosp.I appreciate the complainer that he discussed this matter but he should b so brave to come out and he must do whatever he can do against the doctor. To write down just here should not b enough,he must have to courage to atleast give one slap to a person who tried to abuse him, there is nothing in the world than anyone's respect or dignity.why we dont take steps against any clerk or our president?bcoz sorry to say we r part of that culture, why dont u go to break bad traditions? i m with u , i m with all those persons who go against that kind of act but sorry to say i doubt all this matter

Unfortunately some members forget the actual aim of this forum... Sad

Proof of relationship to your relative living in Canada and evidence that your relative is living in Canada:

Provide official, objective, legible documentation to prove that you are related to your relative. Documents should include birth certificates and marriage certificates. The certificates must state the parents' full names. You must provide certificates for each person that links you to your relative. All documentation must predate your application. Affidavits prepared for the purpose of your application will not be considered conclusive evidence of your relationship.

Provide proof that your relative is living in Canada now. This should include: payslips issued to your relative by their employer in Canada, Canadian income tax documents (recent Notice of Assessment), credit card and bank account statements in your relative's name showing transactions that occurred in Canada. We will not accept utility bills, telephone bills and property ownership documents as conclusive evidence that your relative is living in Canada.

Dear forum members !

I am Dec 2009 applicant and they started processing on my application on 20th Jan, 2012. As I observed that from the starting of processing the people are getting medicals OR request for additional documents within 10-15 days, but in may case I did not yet receive any letter.

Its worrying me. Should I write a mail to CHC London ? What do you people suggest/ advise.

Will be obliged for members' kind time.

Helpfull information thanks.
i also have one more information  if any body want a discounted air fare at landing time then try IOM Worldwide services.
I have following details may be its useful for  some of you
Concession fares are competitive air fares with additional benefits as negotiated by IOM with a wide variety of airline partners worldwide.
What are the main benefits?
The main benefits of concession fares are:
1.   Preferential air fares
2.   Special individual and group booking conditions
3.   Increased baggage entitlement
4.   Simplified payment procedures
IOM-assisted passengers can also benefit from transit assistance at departure and arrival points and transit visa waiver arrangements at selected transit locations where these arrangements have been implemented.
What advantage do I have in using IOM transportation assistance over those of a travel agency for my resettlement or emigration?
IOM offers competitive fares, often well below or on par with locally available market fares with the added advantage of most fares being less restrictive (for instance, possibility of rebooking or even cancellation* without penalty), higher baggage allowance and transit assistance where available.
Who can benefit from IOM's transportation assistance?
Officially recognized refugees in possession of proper documentation, migrants, students and other travellers who fall under a wide category of IOM programmes such as resettlement and returns are eligible for travel under IOM auspices on a temporary or permanent basis.
•   Scholarship holder programmes
•   Agricultural programmes
•   Return of Qualified Nationals
•   Resettlement programmes
•   Special assistance programmes to developing countries, including programmes of technical co-operation with governments.
Does IOM charge a service fee?
Yes, there is a service fee which varies. Please contact IOM's Operational Support Unit (OSU) at osu @ iom.int.
Why are the concession fares not indicated on the IOM website?
In addition to the confidential nature of the negotiated agreements with various airlines, the fares vary according to availability of booking class, seasonality, validity and suitability of routing/itinerary.
From a specific departure point to a specific destination point, there could be a number of choices available. Please contact the OSU Unit (osu @ iom.int) for the fare and routing details suitable to your location.
Can IOM assist me in securing a visa for my country of resettlement?
No, IOM-assisted passengers have to acquire their own visas and other relevant travel documentation before approaching IOM for the transportation arrangements. However in some locations and for some destinations, IOM can assist in the application documentation for a service fee.
Other than a national passport, what other type of travel documents are acceptable for travel?
Before departure, migrants and refugees must be in possession of the following documents:
•   Passport or Travel Document: A valid national passport or other type of travel document (e.g., ICRC or UN travel documents) on which the entry visa to the resettlement country can be affixed. In the absence of any of these documents, the concerned embassies and High Commissions may issue a travel document valid for a single journey as appropriate for their requirements.
•   Entry visa or permanent residence visa
Does IOM assistance include the provision of escorts for special cases?
Medical and non-medical escorts may be needed to assist in specific individual and group departures, up to the port of entry or to the final destination in the country of destination.
Non-medical cases: In some circumstances, unaccompanied minors, elderly refugees or particularly vulnerable persons may require the services of a non-medical escort. In such instances, the IOM office at the point of origin makes available an experienced staff member to accompany the refugee to the destination agreed upon with the destination country.
Medical cases: The conditions for transportation of sick and incapacitated passengers are subject to the rules and regulations of each air carrier involved. Please click here for more details on the requirements for medical cases.
What is the payment procedure?
Payments to IOM for IOM reduced fares shall be made through a bank transfer or a bank deposit. Only in countries where such bank services do not exist may the collection be made in cash.
Will IOM pay for my ticket?
No, IOM will give you access to special reduced fees but cannot pay or subsidize your journey.
Who do I contact?
The Movement Management Department (MMD) based at the IOM Headquarters in Geneva has overall responsibility for all movement aspects worldwide. Within MMD, the Operations Support Unit (OSU) is the focal point for air fares, baggage allowance, transit, airline agreements and all other transport related issues.
Requests are either dealt by OSU directly or channelled by the Unit to the concerned IOM field office.
