Respected Forum members i have a query regarding medical examination,as we have received our medical forms today.kindly suggest me  which DMP we can consult in Lahore as our home station is multan,also tell the procedure to make an appointment and how much medical fee they charge?

can anyone upload these forms so i can match the forms that i have downloaded as i am confused about a couple of them.the forms are as under:-
1- New application for permanent Residence IMM0008 form for the principal applicant.
2- New Schedule A Background/Declaration form
3- New Additional family information form IMM5406

TODAY I GOT REPLY FROM CHC LONDON THAT YOUR APPLICATION HAS BEEN REFUSED DUE TO DUTIES MIS MATCH .WHY I DONT KNOW YESTERDAY I CHECKED MY STATUS WITH ECAS AND IT WAS WRITTEN DECISION MADE WITHOUT GETTING IN PROCESS I DONT KNOW WHY I EMAILED CHC LONDON THAT WHAT IS THIS THEY REPLY ME THAT WE REFUSED YOUR APPLICATION COZ UR DUTIES ARE NOT MATCHING WITH YOUR PROFESSION I AM A FINANCE MANAGER AND CURRENTLY IN LONDON ON THE BASE OF MY JOB WHY CHC WROTE YOUR DUTIES ARE NOT ACCORDING TO YOUR OCCUPATION REALLY I WANT TO LAUGH WTF IS THIS CHC LONDON IS PLAYING WITH APPLICANTS

Hi!

Sorry to hear about your rejection. Did you mention job duties on your experience letter matching to those on HRSDC website for your NOC. I have seen two other cases who have done the same mistake and got rejection.

Hi!

Well I am really sorry and I hope its not rejection. Its that some other people got rejection so I had that in my mind. Any way I am changing my above post. I wish and pray its not rejection. But just to be on safe side I hope you had mentioned correct job duties according to HRSDC website on your experience letter.

Its okay.  Have you seen other people get rejected after status "Decision Made".  Please let me know and any one gotten Medial request after the status change DM.  Yes I have mentioned all duties in that and my consultant also approved that.

Hi Dr. Paro!

I pray its not rejection. Dear yes I have seen members "mhmlhr" and "s786"  in Pakistan application forum thread. Their status changed to decision made few days back without going into in process and without date and yesterday they received rejection letters. In the letter they stated that these people did not mention correct job duties on their experience letter.

mhmlhr admitted he / she never knew he/she had to mention those duties on experience letter and her college gave her the letter so that was her fault.

You can see above ADAN1 got the same rejection. I don't know if he mentioned right duties or not. There is one more applicant in pakistan thread who also got rejection.

I would urge you to send urgent email to CHC London to enquire about your status.

Chances are very minimal close to zero but you can ask for re-consideration with new letter from your employer mentioning correct job duties. But Qorax also advised not to do so as it would be very hard to get decision overturned which has already been decided on basis of documents provided.

If you are very sure that they have made a wrong decision then go for appeal and decision can be overturned, some people have succeeded in it.
But most of the times I have seen that applicants have made mistakes in applications which caused rejection. However in your case u may b right, just review your job duties and consult someone then appeal.
