Overwhelmed! Work Permit process

I am moving to Canada in October from the US. Could someone please help me with a few questions.

My degree is in the health field.

1. Should I apply for an inland application, or outland? Does it make a difference?

2. What do I do with my job offer from a Canadian employer?

3. Where do I get the application to show that I meet the requirements of the Immigration and Refugee Protection Act Regulations?

4. Can I start working without a labour market opinion?
