Hello!

I tried to search for an anwser throughout the forum but didn't find one so thought I'd make a new topic!

Just a few details of our situation:
My husband is American, I'm Canadian. He's currently with me here in Ontario. He has gotten his Medical done in Nov, but that was when we were going to do Inland. Afterwards we found out we should go outland, so we are. My question is, since IMM 1017 is already done, does Appendix C need to be sent to the doctor as well? Also, in the address portion (under Person to be examined) should it be the American address or my address?

When he went for the medical, did the doctor fill out the inland form or the outland form? The forms are different. When you have completed the outland medical, the doctor gives you a green piece of paper to be included with the application. You will need this. If the doctor won't fill it out, he may need to redo his medical.

He should write down his current address, which is his Canadian one if that is where he is living.

The doctor filled out the inland, but while researching, I still need the IMM 1017, don't I? All very confusing haha. I was confused about the address because we're applying outland. So the mailing address is the Canadian one but the residential one is American.

These are all the forms you need. IMM 1017 is for the inland application.

The outland equivalent is a region specific form IMM 3910 appendix C

the doctor will take the appnx C, the sponsoree will fill up the form. and and the exams the doctor will give a green form signed by him/her. ur hubby will submit that form with his application.

in the sponsor's forms u will use ur address, but for ur hubbys forms he must use his address.

Thank you PinkLady!  Grin I just sent that paper to the doctor's office, the only thing that are confusing me now is what it says on the DMP handbook. Which I'm assuming the DMP should know about anyway so I shouldn't be concerned? It just says "The IMM 1017 EFC is used by DMPs for up-front medical exams. It is used when an applicant presents a completed Appendix C – Medical Instructions form." Am I right to assume that this form is at the doctor's office?

I apologize for all the questions, just want to do this all right, this is the last thing we have to do so we're getting antsy haha.

appendix c needs his address. keep on asking questions in this forum. we, the members r here to help each other. good luck to u and your husband.

I don't know about IMM 1017, all I know is when I went for the medical (I did outland while staying in Canada too) all I took was the completed Appendix C which I gave to the doctor. I was given a green paper with my photo attached and this I included in the application.

The address is a tricky one. I believe Suva is right. Mailing address will be his current Canadian address but since he is not yet allowed to 'move' to Canada, his residential address will be USA (I used my parents address in UK)

Thank you both of you! I am a part of Buffalo thread, have been reading it and getting a lot of insight. It's all very frustrating haha. At least this paperwork is the last of it for now! I look forward to the day hubby gets approved and he can work, he's antsy too.
