Ordering CAIPS/GCMS Plz help its urgent

I have following queries about ordering CAIPS .

1. Which is our Ref. No ( Requester's reference No.)
 I'm or my friend, who is the Requester here ?
2.As I'm ordering it through my friend who is a PR of CANADA,
- Should I tick only on Access to information Act (fee $ 5 ) or
- Privacy Act too ??( free )
- Can I also tick for GCMS file ??Will the  File No. (B ...... )  be same for CAIPS and GCMS too ??
- who should  (PA or my  friend in Canada) be signed on the bottom of  IMM 5563 Access to Information and personal information Request form ?
- Should my spouse also fill and sign the IMM 5563 Access to Information and personal information Request form ?
- Should We both ( me & my spouse ) fill and sign the INDIVIDUAL'S CONSENT TO DISCLOSURE
AND/OR USE OF PERSONAL INFORMATION Form ??

2.  No, you want privacy
3.  You tick CAIPS and GCMS
4.  Your friend as you have given him authorization.
5.  Yes, if she wants her information.
6.  Separate authorization for each.

1Should my spouse also need to fill this 5563 form separately for her ,
2or  Only providing authorization separately works ?
3Can I tick the language box that is in privacy Act section ??or should leave language blank ?

i had applied fsw1 noc 0213 in 30 aug, 2009.  but still i am waiting for medical request.  Recently couple of days ago i made general enquiry in which they said my application is in queue for processing.  In case is showing "In process".  What should i do?  Shall i go for case specific enquiry or CAIPS NOTES?  which one I should go first?  Mine is buffalo, ny to new york visa office transferred.

Dear Ram rao, I am also in the same vain like you, e-CAS is showing 'in process' with the same RBVO details. Case specific inquiry will not be valuable in this matter because you've already did general inquiry and got their answers. So, you better request for CAIPS note in my opinion. I am also preparing to request for it. Goodluck to all of us.
