Okay here is a rundown of what to do.. I BELIEVE I am correct, maybe you should PM a "senior" member of the forum for one with authority. Though, if you make a mistake, I am sure the people who send you the CAIPS notes will call you to ensure that you get the right thing done. They called me. Though I included a cover letter explaining what I needed.
1) In the TOP box:
Requestor's reference number/ATIP file number: leave blank
Fill in data for your spouse
"Access to Information Act $5 fee": Tick "Receive copies of records"
"
Privacy Act (no fees)": Leave blank
2) In the big MIDDLE box "REQUEST FOR":
"
CAIPS (Information abroad)": tick box
Write in the embassy (Bogota) and the Visa File number (BXXXXXXXX)
Leave rest of the "
REQUEST FOR" box blank.
3) In the BOTTOM box, tick "
this request is being made by ( ) a Canadian citizen, PR... etc"
Fill in your name and information in the bottom box and sign.
Include a photocopy of the "Use of Representative Form" and a $5 cheque or money order made out to
Receiver General of Canada.
It's just that when I checked out the other thread regarding the CAIPS notes for my own purposes, I noticed that things were different from how they described it, I just followed it and tried to make logical guesses...
