Ordering CAIPS note, some information

Hello there,

I will order CAIPS note for my wife's process to make sure they have received medicals + CSQ. Now, I have searched the forum for some guide on how to do this, but the form seems to have changed.

My wife already filled the "use of a representative" and I am her representative: http://www.cic.gc.ca/english/pdf/kits/forms/imm5476e.pdf
I am her representative, so I guess she doesn't have to fill and sign this form: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5475E.pdf

Is this correct?

Furthermore, I have some questions regarding the filling out the form http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563B.pdf

On the square it says Access to information Act ($5.00 fee), I only check: Receive copies of the record

I do not check anything in the square " Privacy Act (no fees) or do I have to check: English  and "receive copies of the record"? even though they are under the Privacy Act square???

where it says : REQUEST FOR, I check CAIPS (information abroad) and I write Embassy Bogota, Colombia and then her file number starting with B?

Can I send a $5 bill or it has to be a check or a money order from Post Canada for example? Do I have to send a photocopy of my Canadian passport with the papers?

I submitted a request 2 weeks ago and they got back to me cause I was missing things...

You need to submit a copy of the "use of representative" form. We had also submitted one to the visa office after the process began, so that may be why I had to send in a copy (ie. they aren't connected??). Anyway, I had for some reason had my husband sign two copies and had one copy on hand,  she said it was okay if I sent in that one as it is less than a year old.

For the $5, it must either be a cheque or money order only.

As for how to fill out the form, from my understanding, you are correct. As for ID, the form says that you must submit a photocopy of your passport or whatnot if you are submitting this request outside Canada.

that's the thing. the procedure in this thread doesn't seem to apply again.

with that procedure, it says:

Tick the box that says "Access to Information Act ($ 5.00 fee)." ---- I can't tick that, I can only tick: Receive copies of the records.

You HAVE to pay $5.00, there is no way around this. Do NOT tick "Privacy Act" ----  You can't tick that either.

Select which language you wish to receive your notes in, English or French. (These choices are under the "Privacy Act" sub-heading. Then tick the box which says "Receive copies of the Records."

Okay here is a rundown of what to do.. I BELIEVE I am correct, maybe you should PM a "senior" member of the forum for one with authority. Though, if you make a mistake, I am sure the people who send you the CAIPS notes will call you to ensure that you get the right thing done. They called me. Though I included a cover letter explaining what I needed.

1) In the TOP box:
Requestor's reference number/ATIP file number: leave blank

Fill in data for your spouse

"Access to Information Act $5 fee": Tick "Receive copies of records"
"Privacy Act (no fees)": Leave blank

2) In the big MIDDLE box "REQUEST FOR":
"CAIPS (Information abroad)": tick box

Write in the embassy (Bogota) and the Visa File number (BXXXXXXXX)

Leave rest of the "REQUEST FOR" box blank.

3) In the BOTTOM box, tick "this request is being made by (  ) a Canadian citizen, PR... etc"

Fill in your name and information in the bottom box and sign.

Include a photocopy of the "Use of Representative Form" and a $5 cheque or money order made out to Receiver General of Canada.

It's just that when I checked out the other thread regarding the CAIPS notes for my own purposes, I noticed that things were different from how they described it, I just followed it and tried to make logical guesses...
