OPTION C FORM AND T4 SLIPS?!?!?

Hello,
So the "Option C" is called summary of Notice of Assessment or something like that? or is it titled Option C ?!?! I called and ordered it but it doesn't say Option C on it? Its 1 page double sided...is that the right one?
Also, for the Copies of T4 slip do I need for every single year I worked or just for the past Year ( 2010 for example?)

Hi, if you called and ordered Option C, then that sounds right. I also got a one page, two sided document. Mine did not say Option C either. Good luck!

They say it takes 5-10 business days, but it arrived in 5 days for me.
