Hi there,

In Additional Family Information it's required to fill marital status and present address of both parents, my mother and my father got divorced right after I was born, and there is no chance to get this information.  Can I write in those two field something like:"I don't have this information, as I have never contacted my father nor heard about him since my childhood"?

Please advice.

Hi-

Do you know his name or date of birth? (Maybe your mother knows).

Either way, just put in all the fields, "unknown".

You can attach an accompanying letter to the form, and briefly explain the situation.  Since this form is mostly a declaration of family members that you could possibly sponsor in the future, you could say in the letter that you have no intention of ever sponsoring your father.  BUT if you put this and then someday reunite with your father and want to sponsor him, you most likely will not be able to.

who gave you that information. The information could be merely a database to distinguish individuals with same names born on same day and same city. In such situation only way  to separate them is by parent's names

if you dont know anysingle thing about him just fill it with NA.
