Please add her given name in both the "given name" and "family name" spaces. And please do not forget to attach an explanation letter.

And Jimi....And please do not forget to attach an explanation letter. Just mention that since her passport has only given name, and without a familyname you are not able to validate the form, hence you have added her given name in both areas.
