I had the same problem as i don't have a family name, just one big name. Just leave the family name column empty.

Names should be filled as they appear in Passport.

If your wife's passport shows only given name then fill the GIVEN NAMEin the application and leave BLANK in the family name.

Please note that in Europe and America last name is of most importance. Your last name would be called and used in all government and private departments. So if your wife don't have a last name then you should add your first name as her last name but make sure that they wont enter your full name as her last name. Contact your local authorities for changing her record in database. Correct it in her passport and national Id also.

At last i found the suggestion which was given to me from Canada by a Friend.............he said the person who does not have last name in the whole life he or she will face the problems in documentation and always write letters to explain............so he suggested that you must change your National Identity Card record as well make changes in Passport it would be a trouble once but you would save the whole life from troubles........

thanks Hussni you said the right

If you leave either family name or surname column blank, you will not be able to validate your form (as per the new form).

Please include her given name in both the given name as well as the surname column. Please DO NOT add any additional name such as your first name as her last name, and things like that. Since she does not have a family name, just add her given name in both spaces. That's it.

Hope this clarifies your doubts.

Good luck.
