I am truly in a fix and confused, I sincerely don't know which of IMM0008 forms I'm to fill for my added baby in respect to my last post, that is the reason why I requested for your valuable contributions. I don't want to fill and send the wrong one to Accra you know what that means, pls can somebody help me out?

Go to this site  (http://www.cic.gc.ca/english/information/applications/skilled.asp) and open the site, there you will find all the forms but you only need to fill this ones mentioned below. Additional family information form IMM 5406,  Additional Dependants/Declaration [IMM 0008DEP]  (PDF, 472 KB), Supplementary Information - Your travels [IMM 5562]  (PDF, 21 KB) and the payment form for your baby Fee Payment Form – Application for Permanent Residence – Federal Skilled Worker . always follow this forms when ever u want to add a new born baby to ur application

Pls if one gets UCI number after the return your application because the employer did not state well the full duties, task, and periods the employee work with the company what is that UCI number mean and will it affect me if i want to return my application back after getting the right letter explanation from my employer

hi as your application was refused cos of JD, you can't return it after getting the right JD. You can only reapply when the cap resets and the previous UCI will not have any bearing on your new application even though u will have to mention it. So sorry to hear about your situation. What is your timeline?

 @  Uchematt: All of what Mandie said is true except that u'll be required to quote ur UCI number in the IMM0008ENU_2D form during ur re-application. Cheer up as u get ready for the journey ahead.
I've been there & I know it is God that helps one through it.

Pls let me play save by behaving/sounding like an illiterate so as to save me from unexpected and costly embarrassment. I will advice he fills the Schedule A form because its amongst the forms that the VO advised him to fill.

I think the Schedule 3 is not necessary.
