Yes it is normal to ask the travel forms.
You can make a copy of it in MS Excel. Columns headings like SNo., Date of exit, County, Date of entry, Country, stay period. That is it.
rocky272727, my problem is - the CHC, ND has asked me to fill-in the 'Foreign Travel Form' (the one they have attached with the package), but that form is missing! @ mhkhan, many congratulations!!! Wish you all the best! |

Write to CHC and also along with it make the format as I told you on MS Excel and send it to them explaining that if it wont suffice then they can send you the forms again as they were missing. Trust me that is it that they want. So send them the MS excel format history. Best of luck |
