Hallo,

my boyfriend and I have been working on my application for PR status in Canada. When we went down the Document Checklist, we noticed that I never got anything from the doctor. In fact, when we were there, they insisted that we do not use Appendix C as stated in the application guide. Instead we filled out another form. Does this mean that the medical report was not done properly or sent to the wrong office? How do I get that Copy 2 to send it with my application?

You need to contact the DMP who did the examination and ask them to give you the form that confirms that you had the medical examination done.  You need that form, or at least a receipt, to send with your application. 

The DMP sends the results of your medical examination to the Regional Medical Centre that serves the region your processing office is in.  When the processing office is ready to review the medical results, they will request them from the RMC.  The form you submit with your application really only confirms that you've actually done the exam - but if you don't have it, they will write to you as soon as they get your file and ask you to prove you've completed the medical exam.  This will only delay your processing, so get the DMP's office to give you something you can submit confirming your exam. 

They should know these things, but lots of people have had issues and arguments from office personnel in DMP offices who insist they know how it's to be done.  This is because they deal with exams for lots of different types of applicants, and the paperwork requirements are not the same.  Either CIC needs to simplify their process (HA!) or the DMP people need to start listening to applicants instead of insisting only they know what you need.

Married Sept '06 after 4 yr LDR.  Applied via outland ap in March 2007 -  refused Jan '08 due to inadmissible adult "dependent child".  Appeal filed Jan '08; still waiting for resolution.

Thanks for your help.
I tried to get in contact with the DMP's office, but they are closed until the end of July. I guess we'll either have to wait until then or send it after sending the rest of the application.
While we were reviewing all the forms and documents, we noticed that my criminal record check from Australia is already more than three months old. Do you think it will be possible to use this one (since it is a pain in the *** to get another one) and send a note with it saying that I haven't been to Australia since the criminal record check was issued?
