Net worth statement with documents along with medical request- need help

I received a latter from VO on 23rd Oct. the latter state that I do not require attending an interview and must now undergo a medical exam. The later also states that additional supporting documents is required at this time to assist with the review of my documents and ask me to submit (1) Personal net worth statement with supporting documents. , though I have submitted all documents supporting my POF while I submitted full documents to VO (2) Bank statement for last 12 months (3) 5 recent photos of myself and dependents (4) RPRF
Do all of you who received medical request, also received this type of letter? I am having some difficulty in filling in the form of Personal Net worth statement. Would any of you, especially those who have completed this stage, give me suggestion please!
