hi,in my wife's medical report section A (client identification and summary):
There is a option CONTACT ADDRESS/PERSON WITHIN CANADA(NAME,FULL ADRESS AND TELEPHONE NUMBER) in which embassy didnot write anything.So do she has to fill tht space ot it iz not authorized to write anything in medical form?
and in tht form itz written Intended Canadian Destination in which embassy has written PQ wat do they mean by tht?coz I m residing in BC
Guys help me

But I have come to know tht it iz illegal to write anything on medical form section A
