Hello All,

I applied for skilled visa and embassy sent me the medicals forms but envelope lost, now embassy saying I have to download medicals forms from CIC web site I unable to find forms kindly help me How I can download those forms? Please help me as I am very much stuck in middle. Thanks in Advance

Is it possible? Because, the medical forms are case specific with photographs and some personal info of applicants endorsed by CHC.. how could we download it? Please send the query to Qorax or rocky, Belize

Dear Belize,

Don't get unnecessary tensed. There is No Problem with your losing the med forms. That envelope contained the following 3 things:
1. Medical Instructions Letter from the CHC
2. Forms : Medical Report: Section A; Client Identification and Summary [IMM 1017]
3. Forms : Medical Report: Sections B, C, D and E [IMM 5419]


The above forms are 'available' with all DMPs & s/he can do your meds thru that under the 'authority' of the email/letter u recd from your CHC asking u to download the same.

Just make an apponitment with your regional DMP, inform him/her to make your forms & visit him/her for the meds. Your job will be done. Don't forget to carry the following docs when u visit the DMP:
1. CIC sized photos -Qty. 3/head [minimum]
2. Original Passports.
3. That email/letter from the CIC [as mentioned above]

Sec A FORM 1018 is not available on the internet. 
 Those waiting for medical requests and if their mailing address is changed update Visa office at the earliest or you could tend to lose the medical requests and if they do not get the medicals in the time limit specified ur long wait will go in vain as they will just close ur file.

I am a September 2009 applicant. My visa office was Buffalo. I sent full application to Buffalo in Dec 2009 and My application has been transferred to NY office in May 25th and Status has changed to In Process" by Jun 16th

We moved to India from Canada and I had requested for Change of address when my application was transferred to NY office. I sent a mail to NY Immigration office(NewYork-im-enquiry  @  international.gc.ca). But i got the below generic reply from them

" Due to volume of applications, we will only respond to request for application status if processing period posted on our website has been exceeded "

What does this reply means ?, Has my address change request excepted or rejected? Seniors please help me out. I am expecting my Medical request soon, please share ur experience .The Client application status still shows my old Canadian address.

Also i  raised Case specific equiry and still i got the same generic response for the NewYork Consulate
Dont know what to do now.

My landlord at my old canadian address will courier us the forms whenever they receive the same, but still they didnt receive, its almost 1 month after my status changed to "In Process"
Please advice is this timeline normal for NY visaoffice?

Send thru www.cic.gc.ca  CASE SPECIFIC ENQUIRY EVERY 15 dAYS. They will finally update ur address. I did the same.
