Hi guys! Just want to ask where to get the NBI finger form number 5 if the consulate here in Alberta doesn't have one? tnx.

You have to request it from Philippine general consulate. Just google their site and there you could fing the instructions on how to get the form.

If you have a relative in the Philippines, better ask them to send you 1. 1 of my friend did that.
