name policy

hiiiiii every one,
pls. let me know that in imm00008_1e.pdf schedule 1 question no.3 OTHER NAME YOU ARE OR HAVE BEEN USING?......................
AS my name is preeti(as appearing on all my certificates) BUT i have changed it to PREETI GOEL(goel my husband's surname)and now i have made my new passport in the name of PREETI GOEL.
What should i answer to that question?and need to attach any proof for this name changewhile submitting file to CIO?if yes then what proof?
m living in Chandigarh India.

Goel is your surname now and Preeti is your first name but before your marriage what was your first name for example if before marriage you full name was PREETI ABC then you need to write ABC as your old or previous surname, Your latest name is PREETI GOEL as it is your name in you passport so you need to write/ use PREETI GOEL every where and where asked about previous names use your surname before your marriage as previous name, all of your certificates etc. and your marriage certificate contains your old name and your passport bears your latest name so you don't need to worry about proof.
